Dr. Cunningham is a Nominated Medical Adviser under the Queensland Coal Mining Safety and Health Act 1999.
Health assessments (Coal Board Medicals) and 'Section 4' review and completion are available.

 

Information for Employers

Completion of 'Section 1' by employer and issue of 'Section 4' by us (NMA).

The 'Section 4' can only be completed by us (the NMA) upon receipt of a fully completed 'Section 1' from the employer.

When completing 'Section 1' please ensure you complete all sections including the name of the NMA, the SEG and provide comments on positive answers. We will send you the completed 'Section 4' when the appropriately completed 'Section 1' is received.

Delays in issuing the 'Section 4' will be avoided by ensuring all the required information and explanatory comments in the 'Section 1' are provided.

  • Please fax the completed 'Section 1' to us at 07 3868 2063.
  • We will return the 'Section 4' to you by fax.

If you wish to use your own NMA for issue of the 'Section 4'

If you wish a completed health assessment performed by us to be sent to your company's NMA please forward us a request in writing (e-mail or fax) with the NMA's full name, address and fax number. For privacy reasons we will send the medical documents to the NMA only by fax or post.

The legislation prohibits providing any documents other than the 'Section 4' to the employer.

Employers' responsibilities

Employers' responsibilities pertaining to health assessments are regulated by The Coal Mining and Safety Health Regulation 2001, available here (pdf)

These are covered in Division 2, Subdivisions 2 & 3, sections 45 - 49, specifically section 47.

We are happy to discuss these responsibilities with employers seeking clarification or assistance with compliance.
