Information for workers

Dr. Cunningham is a Nominated Medical Adviser under the Queensland Coal Mining Safety and Health Act 1999.
Health assessments (Coal Board Medicals) and 'Section 4' review and completion are available.

Information for workers

We provide health assessments ('Coal Board Medicals') for workers seeking, or applying for, jobs, and workers already working at coal mines. At the completion of the assessment we provide the worker with a completed Department of Mines 'Interim Report' or 'Section 4' as appropriate.

The 'Interim Report' is required by employers who have not yet employed the new worker.

The 'Section 4' can only be completed after the employer has provided the Nominated Medical Adviser with specific information required under the legislation (Section 1). This may need to be done at a time after the completion of the assessment, depending on the employer. The employer must attend to this – it is not the responsibility of the worker.

  • The worker is provided with the required report at the completion of the assessment and does not need to attend to any other issues pertaining to the health assessment.
  • Please telephone us on 07 3268 7411 for appointments.
  • Please bring current photo ID, such as drivers licence.

Expect the medical assessment to take about one hour.

If you wish to see the 'Guidance Notes for completion of Health Assessment' provided by the Department of Mines please see page 1 of the health assessment document by clicking here. The entire document is available for downloading.

If you wish you may complete your part of the document, 'Section 2' only, prior to attending.
